Most marketing agencies stop thinking about a patient the second the sale closes.
They celebrate the booked consult, invoice the practice, and move on to the next lead. If the patient drops off after their first 90-day protocol, nobody notices. If they never refer anyone, nobody tracks it. If they could have been upsold into a deeper program, nobody builds the path.
That's how you end up with a practice on a treadmill — always running ads, always chasing the next new lead, never compounding the patients they already worked so hard to earn.
GROW is the phase that fixes this. It's about the patient journey *after* acquisition — and it's where the real profit lives.
Why GROW is the fifth phase
The math on retention in functional medicine is brutal and under-appreciated.
A functional medicine patient who stays for one 90-day protocol generates maybe $3,000–$5,000 in revenue. A patient who stays for 24 months — multiple protocols, ongoing labs, upsells into longevity or hormone optimization, maybe brings in their spouse — generates 5–10x that. Same acquisition cost. Wildly different lifetime value.
Every dollar invested in GROW returns more than a dollar invested in acquiring a brand-new patient. Most practices don't see this because they aren't measuring it.
GROW is how the practice compounds. It's also where my guarantee — 10 new patients per month or the client doesn't pay — becomes sustainable. A practice built on acquisition alone can't keep up with that pace forever. A practice with a working GROW phase accumulates compounding returns that make the acquisition math easier every month.
The four GROW letters
### G — Guided Support (AI App)
Between visits, functional medicine patients are largely on their own. They've got a protocol, a supplement list, maybe some dietary changes, and 30 days until they see the doctor again. A lot can go wrong in 30 days.
A guided support layer — often an AI-powered patient app — checks in, tracks symptoms, flags deviations, and answers routine questions without requiring the doctor's time. This does two things:
1. Clinical outcomes improve because patients stay compliant and course-corrections happen early
2. Retention improves because patients feel supported instead of abandoned
The practice becomes "the one that actually helps between visits," not "the expensive doctor who does one call every 30 days."
### R — Retention Sequences (Active Treatment)
Patients in the middle of a protocol need sequenced communication that reinforces they're on the right path. Not sales emails. Not generic newsletters. Content cued to the specific stage of their treatment:
- Week 2: What to expect as protocols take hold
- Week 4: Mid-protocol check-in, symptom journaling prompt
- Week 6: Preparing for the first retest labs
- Week 8: Reading your retest results before your next visit
- Week 12: What a typical 3-month review looks like
This keeps the patient engaged during the inevitable mid-protocol slump, when results are incremental and motivation flags. Without this layer, churn spikes around week 6.
### O — Ongoing Care (Post-Treatment)
The moment most agencies stop: the protocol is complete, the patient is "done." In functional medicine, done is rarely done. Most patients benefit from:
- Quarterly labs to monitor maintenance
- Seasonal protocol adjustments
- Annual deep re-work as life circumstances change
- Complementary protocols (hormones, gut, mitochondrial — the conditions that feed into each other)
Ongoing Care is the automation that invites patients back in at the right rhythm — not pushy, not salesy, just reliable. Every returning patient is an acquisition cost saved.
### W — Win-Back Systems (Upsell + Re-engagement)
For patients who churn anyway — they moved, their finances changed, they plateaued — Win-Back is the sequence that keeps the door open:
- 60 days post-last-visit: "How are you doing?" check-in
- 90 days: A relevant new insight or research note
- 6 months: A re-engagement offer
- 12 months: Anniversary message with a new service option
Most practices never send these. The ones that do find that 15–25% of churned patients come back within 12 months. That's free revenue that would otherwise be left on the table.
Win-Back also includes upsell paths for active patients — longevity programs, family packages, group cohorts. Every patient already in the door is a warmer lead for a deeper offer than any cold lead.
The most common GROW mistake
Building GROW last.
Most practices don't start GROW work until they feel like they have "enough" active patients to justify the effort. By then, the early patients have already churned. The retention data is polluted. The compounding never starts.
GROW should be built alongside CLOSE, not after. The moment a patient commits, they should enter a structured retention path. Most practices can build functional versions of all four GROW letters in 4–6 weeks. The effort is small compared to the returns.
